Your 7-Month Initial Enrollment Window

When you turn 65, you have a 7-month Initial Enrollment Period to sign up for Medicare. This window starts 3 months before your 65th birthday month, includes your birthday month, and extends 3 months after.

Timeline Example

If your birthday is June 15th:

  • March 1 - May 31: Early enrollment period begins (best time to enroll)
  • June 1 - June 30: Your birthday month
  • July 1 - September 30: Final 3 months to enroll

Important: When Coverage Starts

If you enroll in the 3 months before your birthday month, coverage starts on the first day of your birthday month. If you enroll during or after your birthday month, there may be a delay in coverage starting.

Medicare Checklist: 3 Months Before Your 65th Birthday

Step 1: Determine If You Need to Enroll

Still working with employer coverage? If you have coverage through an employer with 20+ employees, you may be able to delay Medicare enrollment without penalty. However, this requires careful coordination.

Receiving Social Security? If you're already receiving Social Security benefits, you'll be automatically enrolled in Medicare Parts A and B. You'll receive your Medicare card in the mail about 3 months before your 65th birthday.

Not receiving Social Security? You need to actively enroll in Medicare during your Initial Enrollment Period.

Step 2: Decide Between Original Medicare or Medicare Advantage

This is one of your most important decisions. You have two paths:

  • Original Medicare (Parts A & B): Maximum flexibility to see any doctor who accepts Medicare nationwide
  • Medicare Advantage (Part C): All-in-one coverage through private insurance companies, often with extra benefits like dental and vision

Step 3: If Choosing Original Medicare, Add Supplemental Coverage

Original Medicare has gaps in coverage. Most people add:

  • Medigap (Medicare Supplement): Covers copays, coinsurance, and deductibles. During your Initial Enrollment Period, you have guaranteed issue rights (can't be denied).
  • Part D (Prescription Drug Coverage): Protects you from high prescription costs. With the 2026 $2,100 out-of-pocket cap, Part D is more valuable than ever.

Even if you don't take many prescriptions now, enrolling in Part D during your Initial Enrollment Period avoids a 1% penalty for each month you go without coverage (unless you have creditable drug coverage from another source).

Waiting Too Long to Shop for Medigap

Your Medigap Open Enrollment Period is the 6 months starting when you're 65 and enrolled in Part B. During this time, you can't be denied or charged more based on health conditions. After this window, you may face medical underwriting.

Not Checking If Your Doctors Accept Medicare

Before enrolling, verify that your current doctors in Lakeland accept Medicare or are in-network for the Medicare Advantage plan you're considering.

Special Situations for Florida Residents

Working Past 65 with Employer Coverage

If you retire before 65 and lose employer coverage, you'll need health insurance until Medicare starts. Options include COBRA, ACA marketplace plans, or your spouse's employer coverage.

  • Part A: $0 for most people (if you or your spouse paid Medicare taxes for at least 10 years)
  • Part B: Standard premium is approximately $185/month (exact amount set annually)
  • Part D: Varies by plan, typically $0-$100/month
  • Medicare Advantage: Many $0 premium plans available in Lakeland (plus Part B premium)

High Income Surcharges (IRMAA)
